    VENDOR INSURANCE REQUIREMENTS FOR
    DALLAS KOI KICHI LABOR DAY KOI SHOW
    AUGUST 30, 31, SEPTEMBER 1, 2013
    AT GRAPEVINE BASS PRO OUTDOOR WORLD

    Due to the high profile of Bass Pro with thousands of people passing through their properties each weekend, they are highly risk adverse and have given us certain insurance parameters we and our vendors must meet to participate at this year's show.  Please review the liability insurance requirements below and complete the information.  If you have any questions, please email Tom McWhirter, DKK insurance coordinator, at cmcw66@hotmail.com.  We think this is a small price to pay for the high visibility location and traffic count at the Bass Pro location in Grapevine.
     
    We will need your insurance information sent to Tom McWhirter at cmcw66@hotmail.com by August 23, 2013.  This is a firm date as he must submit insurance requirements to Bass Pro by August 23, 2013.   We will appreciate your promptness in this regard.
     
    If your company insurance does not comply with Bass Pro's insurance requirements, we have a broker that will issue the correct liability insurance policy.  You can simply add another $50 to your booth fee and DKK will obtain this policy for you or if you wish to work directly with our broker, contact Tom McWhirter and you will receive contacts and phone numbers (cmcw66@hotmail.com, phone 972-267-8336). 
     
    The following insurance is required by Bass Pro if you wish to participate in the DKK Koi Show:
     
         1. Commercial General Liability (GCL) - $1MM per occurrence and $2MM annual aggregate.
     
         2.  A copy of your current automobile insurance coverage, either personal or commercial policy.
     
         3.  Worker's Compensation per state's statutes.  There is a gap of coverage in the GCL that does not cover vendor employees at the koi show.  You are exempt  from this worker's comp requirement if you are a one person operation and not bringing employees.  We just need you to identify attendees on the Special Events Agreement (SEA) that we will be emailing to you.  If exempt from worker's comp, please send a statement indicating your exemption.
     
    Please advise your insurance broker of the following:
     
         1.  The additional insured should be Bass Pro Outdoor World, L.C., it's parents, subsidiaries, affiliates, employees, officers and directors, shall be named as Additional Insured on all insurance policies.
     
         2.  The certificate holder should be:

    Thank you for participating in our 2013 Dallas Koi Show.  We have a great venue in our partnership with Bass Pro Outdoor World.  Please have all the special event insurance required above submitted by August 23, 2013.  Bass Pro has advised us that all vendors must be pre-approved to participate in this year's event at their Grapevine location.
